So how is this different than other street medicine programs?

Our Integrated Health Outreach (IHO) is unique in that we staff our outreach teams to adapt to our client’s complex needs, which are usually more than just medical care. Sometimes it’s housing, or harm reduction, or resource navigation, crisis management, or even dental! We staff our teams to be prepared to meet whatever needs the client expresses, wherever they need it.

Where do you outreach?

Our IHO team delivers low-barrier care to people experiencing homelessness via roving outreaches to encampments and other client-occupied public spaces all throughout Bernalillo County. Additionally, our outreach teams meet clients on a weekly and rotating basis across seven site-based locations, including Gateway West, also known as the Westside Shelter.

When do you outreach?

We outreach over 25 hours a week! Each day we focus on a different part of our service area. To check out where we’ll be and when, take a look at our outreach calendar.

I don‘t think I’ll be able to come into the clinic, can you come see me in the community?

Yes we can! You can call our receptionist and schedule an outreach visit. Just let us know your name and where you’ll be and when. Based on our outreach schedule and where we’ll be, we will try to see you as soon as possible!

What Outreach Looks like at AHCH

The PIT Count

Each year, communities across the country participate in the federally required Point-in-Time (PIT) Count — a one-night survey intended to estimate the number of people experiencing homelessness. In Albuquerque, the PIT Count helps guide the allocation of critical federal, state, and local funding for housing, shelter, outreach, and supportive services. The count also creates meaningful opportunities for volunteers to engage directly with the community by helping conduct surveys and outreach alongside local organizations.

While the PIT Count is an important planning and advocacy tool, it is widely understood to be a severe undercount of the true scale of homelessness. Because it captures only a single night in time, many people experiencing homelessness are missed, including those staying temporarily with others, living in vehicles, or moving between unstable situations. In 2025, Albuquerque’s PIT Count identified 2,960 people experiencing homelessness, a 120% increase in just 2 years, yet Albuquerque Health Care for the Homeless alone serves more than 5,000 unique individuals each year. Many advocates estimate that the true number of people experiencing homelessness may be several times higher than the annual count suggests.

Outreach Challenges

Homelessness in Albuquerque continues to grow, while the systems designed to support people remain stretched beyond capacity. With at least 3,000 individuals experiencing homelessness and fewer than 2,000 shelter beds available across the city, many people simply have nowhere else to go. As encampment sweeps and displacement efforts increase, outreach teams are finding it harder to locate and consistently care for individuals who are already exhausted, traumatized, and working hard to survive day to day.

When encampments are cleared, people often lose track of vital belongings including medications, identification documents, clothing, survival supplies, and connections to care. Rather than solving homelessness, displacement frequently scatters people farther from services and makes ongoing health care, outreach, and case management more difficult. Outreach helps people survive and builds trust over time — but when people are repeatedly pushed from place to place, it becomes harder to deliver the consistent support needed to move toward long-term stability and housing.

Get Involved

Ending homelessness takes all of us. There are many ways to support your unhoused neighbors and help build a stronger, more compassionate community. You can get involved by volunteering, donating critical needs items, or joining outreach and Point-in-Time (PIT) Count efforts each January alongside AHCH teams.

Small acts of kindness also matter deeply. Carry extra bottles of water during extreme heat, check in on people who may be struggling, and treat every person with dignity and respect. If you encounter someone who may want support or services, you can help connect them to care by reaching out to AHCH. Most importantly, raise your voice — advocate for housing, healthcare, and policies that address the root causes of homelessness and create lasting solutions for our community.

Outreach is also one of AHCH’s most underfunded programs. There is little to no public funding available for many outreach supplies and survival resources, making community support essential to sustaining and expanding this work. Monetary donations help AHCH continue to provide water, hygiene supplies, wound care, transportation assistance, survival gear, and direct outreach services to people living outside — helping meet immediate needs while building pathways to long-term care and housing.
