AHCH partners with local emergency shelters to bring health care and supportive services directly to people experiencing homelessness. Our Community Health Worker outreach teams connect shelter guests with medical care, behavioral health services, case management, housing resources, and other essential supports, helping remove barriers to care and ensuring people can access the services they need on their path toward health and housing.
