AHCH’s Integrated Health Outreach team brings integrated health care, harm reduction services, and support directly into the community. Through our outreach model, community health workers, harm reduction specialists, and clinical nursing staff connect with people experiencing homelessness throughout Albuquerque to provide compassionate care, health education, supplies, resources, and connections to ongoing services. By meeting people where they are and building relationships rooted in trust, outreach helps reduce barriers and creates pathways to improved health and stability.
