AHCH partners with Gateway West, also known as the Westside Shelter, to bring integrated health care directly to shelter guests through our on-site pop-up outreach clinic. Our multidisciplinary team—including medical prescribers, registered nurses, and community health workers (CHWs)—provides primary care, acute medical services, medication management, health screenings, and connections to behavioral health, housing, and other supportive services. By meeting people where they are, we reduce barriers to care, address health concerns early, and build trusted relationships that connect individuals to ongoing care and long-term stability.
