Primary Health Care
Health care is more than treating illness—it’s about caring for the whole person. AHCH provides integrated, trauma-informed primary care designed specifically for people experiencing homelessness. At our Downtown Health Care Campus, patients can access comprehensive services including medical, behavioral health, dental, optometry, pharmacy, and specialty care. We reduce barriers to care and make it easier for patients to receive coordinated, whole-person care. All of our services are portable, so we can also take our integrated health model to the streets of Bernalillo County.
For patients who are too ill or injured to recover on the streets but no longer require hospitalization, the Albuquerque Medical Respite Collaborative Program offers a safe place to heal. Through our partnership with the City of Albuquerque, Heading Home, University of New Mexico Hospital, and First Nations Community Healthsource at the Gateway Center, patients receive short-term recuperative care from an interdisciplinary team, along with case management, housing navigation, and connections to ongoing health care and community resources. Medical respite provides the time, support, and stability needed to recover with dignity while reducing unnecessary hospitalizations and helping patients transition toward improved health and housing.
Social Services
Stable housing, access to benefits, and strong support systems are essential to good health. AHCH’s social services team helps patients navigate housing resources, obtain identification documents, access public benefits, connect to community resources, and overcome barriers that impact their well-being. Through compassionate, person-centered support, we help individuals build pathways toward stability, independence, and belonging.
Outreach
Care doesn’t begin when someone walks through our doors—it begins wherever they are. AHCH’s outreach teams bring integrated health care, harm reduction services, and supportive resources directly into the community and conducting street outreach throughout Bernalillo County. Through nurses, primary care providers, community health workers, and harm reduction specialists, we build trusting relationships, provide essential care, and connect people experiencing homelessness to the services they need on their journey toward health and housing.
