For more than 40 years, Albuquerque Health Care for the Homeless has been leading the way in compassionate, person-centered care for people experiencing homelessness and housing instability.
As New Mexico’s only stand-alone Health Care for the Homeless program and one of the nation’s original 19 federally funded Health Care for the Homeless projects, we have spent four decades demonstrating what is possible when health care is rooted in dignity, human rights, and belonging.
Today, we are a Federally Qualified Health Center and nationally recognized Patient-Centered Medical Home providing integrated medical, behavioral health, substance use treatment, case management, housing support, and specialty services under one roof. Our work is grounded in trauma-informed care, harm reduction, and Housing First principles that recognize every person’s inherent worth and potential.
But our mission extends beyond health care alone. We are committed to creating exceptional care experiences, amplifying the voices of people with lived experience, and advancing the policies and systems needed to end homelessness. Every day, we work alongside our clients, partners, and community to build a future where everyone has the opportunity to be healthy, housed, and thriving.
